Permission slips are required for the safety of each student.
Please read below to choose which form you need.
Special Events Permission Slip. This permission slip is required of all people attending a special Lighthouse Youth event. On the calendar or event flyer a statement will indicate "Permission Slip Required." Please click this link and print the permission slip. When you have completed it, please bring it with you to the special event.
Required Annual Permission Slip. This permission slip is required of all who participate in any Lighthouse Youth events. These include Impact, small groups, Friday Night Live, and others. Please click this link and print the slip. When you have completed the form, mail the slip to: Dewayne Kirkman, First Presbyterian Church, 404 Spruce Street, Elk Rapids, MI 49629.
